CMS’ New Transforming Episode Accountability (TEAM) Model for Hospitals

TEAM is a newly finalized five-year, episode-based alternative payment model. Mandatory for acute care hospitals within selected Core-Based Statistical Areas (CBSAs), spanning 30 days. It will cover the acute care hospital setting and the post-discharge period. Our speaker will discuss the TEAM Model in more detail and the opportunities it poses for SNFs and post-acute care facilities.
